Participation Waiver & Release

I understand that participation in music activities, karaoke, crafts, face painting, instrument exploration, food service areas, and related event activities involves normal risks associated with children’s and family activities. I understand that food and beverages may be available during the event and acknowledge that Desert Ridge Music Academy cannot guarantee an allergen-free environment. I accept responsibility for monitoring my child(ren)’s dietary needs and allergies. On behalf of myself and my child(ren), I voluntarily assume these risks and release and hold harmless Desert Ridge Music Academy, its owners, employees, instructors, volunteers, and affiliates from liability for minor injuries, accidents, losses, or damages arising from participation in this event, except in cases of gross negligence or intentional misconduct.

I certify that my child(ren) are physically able to participate in event activities.

Media Release Notice

I understand that photographs and videos may be taken during the event and may be used by Desert Ridge Music Academy for promotional purposes including social media, website content, newsletters, advertisements, and future marketing materials. By attending this event, I grant permission for my child(ren)’s image and/or likeness to appear in such materials.
